Required Forms for Student Participation in Athletics & Other MSHSAA Activities

***Print off either English or Spanish pages of the Required Physical Forms 

and complete prior to your physical.  

Turn in to your coach or Athletic Director before first day of practice.


For all CHS athletes and students involved in band or music activities to participate in MSHSAA sponsored activities, a current physical, drug testing consent paperwork, and proof of insurance must be on file in the Athletic Director's office prior to the first practice of the season.


A free physical night is scheduled each spring. The next one will be Tuesday, May 22, 2018 from 6 to 7:45 pm.  Bring the completed paperwork above. Make sure all parent signatures are made before getting in the exam line.  These physicals will cover all athletes for the 2018/19 school year.



Please insure that the doctor's signature, student and parent signatures are made in all locations

as needed by the stars. Please take the time to read all documents before signing.


Each athlete is required to have medical insurance.  If your family does not have coverage,

our school district offers access to this K&K Insurance Policy for purchase.  

Must be done prior to the first day of practice. On-line sign-up is available with credit or debit card.

Carthage R-9 Athletic & Activity Policies

A mandatory spring sports parents meeting with the athletic director and spring coaches is scheduled for March 5 at the High School commons area at 7:00 p.m.
